Required fields
The guidance matters more than the field name. A field labelled “risks” with no guidance gets filled in with “standard AI risks” and the artifact stops being worth reading.

Worked example
Filled in against the sample record set, so the template can be judged on what a real entry looks like rather than on its headings.
Program status — August 2026
- Decisions needed: four at the September 15 committee (two pilot dispositions, one vendor checkpoint, one disclosure practice). None require leadership action ahead of that meeting.
- Portfolio: 10 registered use cases, 4 pilots underway, 1 paused, 5 tools inventoried, 3 exceptions active of which 2 expire within 30 days.
Outcomes
- Measured: 268 monthly active users, up from 48 in January. 64% license utilization. Median 3 business days from intake to pathway assignment.
- Measured: approximately 180 hours returned, validated by timed sampling against a pre-pilot baseline.
- Estimate: approximately 820 hours returned in total, extrapolated from measured per-task reduction across observed volume.
- Self-reported: program perceived as helpful rather than blocking, 3.9 of 5, n=142, up from 3.4.
- Pending validation: all four measures on the recruiting coordination pilot, which starts 2026-09-08.
What we do not yet know
- Whether hours returned are being redeployed to higher-value work or absorbed. Nothing in the current measurement answers this.
- Whether the 1,098 employees who did not respond to the program survey hold a different view from the 142 who did.
- Whether PIL-002's false-positive rate can reach the threshold, or whether the ceiling is structural.
- Risks and blocked: ACT-043 blocked on vendor documentation since April; it gates EXC-02, which expires 2026-10-15 and covers two active pilots.
